Licensed Addiction Counselor is responsible for implementing and operating a program of addiction services offered at Providence House. Programs and services provided will align with the mission of our organization and will comply with all regulatory requirements. Some of the services provided by Providence house include adult residential treatment, outpatient treatment, group and individual counseling, alcohol/drug evaluations, and treatment planning.
Responsibilities and Duties
Evaluate patients for substance use history.
Use this information to document signs, symptoms, physical, mental, emotional, social, and spiritual implications, and treatment recommendation.
Develop and execute treatment plans.
Coordinate care with other providers and insurance companies to share observations and recommendations as well as re-evaluate patients to determine progress throughout care.
Monitor patient adherence to treatment plan and determine if goals are being met.
Provide education and addiction group sessions.
Document timely and accurately the patient assessments, evaluations, and therapy notes.
Obtain releases of information and acknowledge CFR 42 laws when sharing recommendations with client, family/guardian, and other care providers for feedback.
Periodically measure the results of the programs and services against program goals. Implement program adjustments as needed to enhance services.
Attend daily clinical staffing meetings to evaluate residents progress.

Providence House is a not-for-profit organization founded by the Sisters of St. Joseph that helps transform the lives of homeless, abused, and formerly incarcerated women and their children one family at a time. We provide case management and our services and programs provide shelter and support through our network of transitional residences, individual apartments and permanent supportive housing located in Brooklyn, Queens and New Rochelle, New York.
