Responsibilities:

• Providing limited term skilled nursing services to individuals returning home from higher level care facilities

• Assessing, generating, and evaluating patient care plans and patient progress

• Liaising with other members of each patients multi-disciplinary care team

• Recording patient progress in HCHB via tablet

• Attending regular staff meetings

Requirements:

• Current NJ RN license

• Reliable transportation, drivers license, and insurance

• Strong interpersonal and customer service skills

• Ability to demonstrate knowledge of electronic documentation

• Additional training certificates a plus

• Preference for nurses with prior Home Health experience including: Psych, Vent, Peds, Admissions

• Monday to Friday
• Rotating weekends

Experience:

• Nursing: 1 year (Preferred)

License/Certification:
