Description

Responsibilites: 

  • Provide care coordination activities for clients.
  • Assuring that member has access to, engages in and retains needed services as defined in the members care plan. Such services may include: Acute Medical Care, Primary Medical Care, Preventative medical care Services, Home Health Care, Behavioral Health Services, Community Social Support Services,
  • Housing; State and Federal entitlements.
  • Advocate for client services by phone and in person as needed.
  • Perform home visits and follow-up to monitor services and client status.
  • Participate in case conferences and trainings.
  • Maintain relationships with service providers.
  • Maintain accurate and timely documentation including all documents in client charts, records and billing activity logs.
  • Attend supervision with Program Director as per agency policy.

Qualifications


  • Requires a Bachelors Degree or a High School Diploma with 2 years of care management experience.
  • Experience working with the homeless, and HIV populations required.
  • Bilingual preferred, with good verbal skills in Spanish.
  • Must have exceptional communication skills both orally and in writing, with individuals and groups regarding complex and sensitive issues.
  • Clean valid driver's license required.
