We are currently searching Registered Nurses for Alameda (specifically Zone 3 - San Leandro, San Lorenzo, Hayward, Union City, Fremont, Alameda, Castro Valley, Bay Farm, Livermore, Pleasanton, and Dublin) that specializes in Home Health nursing to provide patients with general care services. The ideal candidate is an experienced Nurse who also has a passion for caring for the elderly and will work efficiently in a fast-paced, home environment. This professional will work directly with family members to ensure that the patient is able to maintain their quality of life. The Registered Nurse will also be involved in interdisciplinary decision-making that can alter the health of their patients.

Responsibilities:

• Determine if the patient is taking medications correctly and if they are working.
• Observe the patient to make sure that they are not getting worse. Discuss pain management strategies with the patient and family.
• Assesses patient needs and develops plan of care with physician.
• Coordinates and oversees care plan with patient, family, and support staff—LVNs and nurse assistants.
• Monitors and evaluates patient response to treatment, medication, and healing or mobilization progress.
• Oversees case management.
• Administers medication and intravenous infusions.
• Takes vital signs, draws labs, and manages wound care.
• Tends to wounds and other needs of home health patients.
• Works with clinical healthcare professionals to coordinate home care.
• Performs physical assessments.

Home Health Registered Nurse Qualifications:

• Valid RN License to practice in CA
• Current BLS/CPR certification
• Valid CA Driver’s license
• Minimum of one year nursing experience is preferred

Job Type: Full-time

Pay: $80,000.00 - $110,000.00 per year
