Job Responsibilities:

• Provide direct patient care under the supervision of a registered nurse
• Follow the plan of care, provide treatments, and work collaboratively with the members of the team to help meet positive patient care outcomes
• Implements plan of care initiated by the registered nurse
• Assesses and provides patient and family/caregiver education and information pertinent to diagnosis and plan of care
• Participates in coordination of home health services, appropriately reporting the identified needs for other disciplines (Home Health Aides, Occupational Therapists, Physical Therapists, Medical
• Social Workers, Speech Therapists, Dietician) to the registered nurse and or Clinical
• Supervisor/Nursing Supervisor

Qualifications:

• Valid LVN License to practice in CA
• Current BLS/CPR certification
• Valid CA Driver’s license
• Minimum one year Clinical experience as LVN/Home Health is preferred
