We are seeking a dedicated and compassionate Hospital Liaison to join our Progressive Quality Care team. As a Hospital Liaison, you will play a crucial role in connecting our facilities with hospitals, medical professionals, and community partners. Your primary responsibility will be to foster positive relationships and ensure seamless communication between our organization and external stakeholders.
Hospital Partnerships: Establish and maintain strong relationships with local hospitals, clinics, and medical centers. Collaborate with their staff to facilitate patient transfers, coordinate care, and streamline processes.
Referral Management: Serve as a point of contact for hospital discharge planners, case managers, and physicians. Receive referrals, assess patient needs, and ensure a smooth transition to our facilities.
Community Outreach: Engage with community organizations, senior centers, and other healthcare providers. Promote our services, educate potential patients and families, and participate in health fairs and events.
Patient Advocacy: Advocate for patients’ best interests during transitions. Address any concerns, answer questions, and provide emotional support to patients and their families.
Documentation and Reporting: Maintain accurate records of interactions, referrals, and outcomes. Generate reports on referral trends and performance metrics.
Collaboration: Work closely with our admissions team, nursing staff, and administrative personnel. Communicate effectively to ensure efficient patient admissions and discharges.
