Prime Home Health Position: Registered Nurse (RN) Case Manager/Clinical Supervisor

• Seeking Full-time RN Case Manager

Reports to: Director of Patient Care Services

Job description: We are looking for dedicated RNs who want to provide quality care to home based patients. You will work one-on-one with the patient and truly impact their life as well as their families. The job is consistent, long-term, and can work with a variety of different schedules. The RN is responsible for providing direct patient care under the supervision of the Director of Nursing. Responsibilities include creating the plan of care, conducting monthly onsite visits, training and orienting LVNs, and working collaboratively with the members of the team to help meet positive patient care outcomes. This is an office-based position. Aside from conducting on-site visits, the RN Clinical Supervisor is expected to work at their office base during regular business hours. The RN Clinical Supervisor will be overseeing LVN patient home care which includes Patient home visits, creating and updating Plans of Care, training and orienting LVNs, and conducting initial patient assessments.

REGISTERED NURSE (RN) RESPONSIBILITIES:

• Provides direct patient care as defined in State's Nurse Practice Act
• Creates patient Plan of Care and updates at recertification.
• Provides accurate and timely documentation consistent with the plan of care
• Assesses and provides patient and family/caregiver education and information pertinent to diagnosis and plan of care.
• Prepares and coordinates clinical notes and progress notes.
• Weekly QA review on patient care notes
• Conduct weekly New Hire orientations as needed.
• Manage a caseload of patients
• Conduct on-site field visits on a monthly basis, and as needed.
• Communicate regularly with patient care team as needed.
• Participate in clinical on-call rotation REGISTERED NURSE (RN)

QUALIFICATIONS:

• Current CA Registered Nurse License, 1 year experience required.
• Current PPD or chest x-ray if applicable
• Current State license, CPR certification and valid driver's license (local travel required)
• Must be available during our business hours Monday- Friday 8am-5pm We welcome you to submit your resume

Pay: $80,000.00 - $85,000.00 per year
