We are looking to add a LVN with Wound Care experience to their fast growing practice in the Golden Triangle area. The LVN will conduct home visits in conjunction with nurse practitioner and/or physician in the patient’s place of residence.

Duties include:

• Coordinates wound care therapy into the total nursing care plan for patients;
• Participates in patient and family teaching and provides leadership by working cooperatively with ancillary nursing and other patient team personnel to maintain standards for professional nursing practice in the clinical setting.
• Assesses the medical status of assigned patients;
• Prepares accurate documentation of the visit in a timely manner;
