Pride Health is looking for an Assessment Registered Nurse, you will visit patients' homes and complete appropriate assessments, and documentation, assist with enrollment, service needs, etc.

Duration: 6 months contract

Duties:

• Conduct assessments for prospective and current members, leveraging clinical expertise.
• Perform in-person Care Management visits to facilitate person-centered service planning.
• Assess members’ changing needs.
• Review nursing assessments and medical notes to adjust Care Plans or assist with nursing home placement.
• Coordinate with home care agencies, vendors, pharmacies, and medical providers based on identified needs.
• Provide community-based education, emphasizing self-care and prevention.
• Handle additional care management tasks as assigned.
• This position is 100% field work

Qualifications:

New York State RN license Required

Ability to travel in and around Bronx, New York Required

Active NY state driver’s license or NYS ID Required

Bilingual Spanish/English Required

Field experience assessing, planning, and evaluating members'/patients' care. Highly Preferred

Electronic patient health information (PHI) database experience Highly Preferred

Knowledge of health insurance, Medicaid, Medicare, and MLTCP Highly Preferred
