The Stop Now And Plan (SNAP®) F.A.T.H.E.R.S Program serves as a “front-end” resource to the Department of Juvenile Justice Office of Prevention for Fathers of children between the ages of 6 and 12, and their families.
¨ The SNAP® program provides high-risk youth and their families strategies to increase pro-social skills that will help the youth stay in school and out of trouble by making better choices throughout the 13-week program. Youth and their families participate in engaging activities such as group discussions, role-playing, interactive games and self-reflection to address topics including dealing with anger, learning how to cope and practice self-control, engaging in problem solving and learning not to bully and how to prevent bullying.
“Fostering responsible fathers to build strong families and safe, thriving communities to halt the intergenerational transmission of adverse childhood experience (ACE) exposure and justice system involvement”.
SUMMARY:
We are looking for Case Manager to join our growing team!
DUTIES AND RESPONSIBILITIES:
· Responsible for the day-to-day case management of the F.A.T.H.E.R. initiative, including SNAP® Girls and Boys.
· Completes staff training as required by SNAP® and agency.
· Completes intake assessments for SNAP® Girls and Boys including all required pre and post assessments.
· Follows up on incoming referrals and schedules clients for intake appointments.
· Monitors youth and family progress through weekly compliance calls.
· Liaises with relevant community agencies to include schools, community-based agencies, and police departments to enhance outreach.
· Completes data reporting requirements for intake assessments and ongoing weekly data reporting requirements to all
relevant systems in a timely manner.
· Ensures weekly materials are prepared for SNAP® groups (handouts, manuals, and all other required items).
· Utilizes a strength-based perspective to help youth and families strengthen and manage the quality of their lives by engaging the family, advocating on their behalf, initiating change agent activities, helping them access supports in the community, teaching problem solving skills, modeling productive behaviors, and assisting in the successful completion of youth and family developmental milestones.
EDUCATION and/or EXPERIENCE:
· A bachelor's degree in social work or a degree in a related field with at least two (2) year of experience in working with
children and families and experience in case management.
Qualifications and Skills:
· Passion for intervention.
· Experience working with at-risk children and families.
· Ability to communicate effectively with diverse group of individuals.
· Ability to demonstrate active listening, validation and patience in a group setting.
· Good verbal and written skills or willingness to learn.
· Good interpersonal relation skills or willingness to learn.
· Successfully pass a Level II Background Screening.
Expectations:
· Attend a 5-day Core SNAP Training.
· Participate in the intake and assessment process as needed.
· Demonstrate a commitment to the SNAP programs and values.
· Participate in a minimum of two 13-week cycles.
· Participate in weekly de-brief and ore-briefing sessions.
· Participate in additional SNAP meetings as necessary.

organized to address the issues over representation of minorities in the juvenile justice system. Our services are designed to work with runaway, truant, and ungovernable youth and their families by increasing social and life management skills, academic success and self-worth awareness. We expanded to respond to issues of Family Reunification Services, Prevention, Emergency Food Pantry, Parenting and Neighborhood Empowerment. Since inception, our focus has been on the needs of disadvantaged youth and the families in the inner city of Fort Lauderdale and Broward County as a whole.
