This position pays up to $65,000 per year depending upon experience!

Join the Atlantic Shores Rehabilitation and Healthcare Center family in Social Services where you will provide patients/residents with the highest practical level of physical, mental, and psychosocial well-being and quality of life. You will identify through communication with patients/residents, their family members/significant others, and nursing center staff, patient/resident's strengths, social, emotional, and mental health needs' and then provide, develop, and/or aide them in accessing services to meet them.

Position Highlights

• The Social Worker collaborates with the interdisciplinary team to promote and protect resident rights and the well-being of all patients/residents. This includes educating staff on the role of the social services
• The Social Worker participates in the development of a written, interdisciplinary plan of care for each patient/resident to address social services needs
• The Social Worker holds meetings with patients/residents, families, and significant others as requested, to provide support and information to improve quality of life
• The Social Worker performs patient/resident discharge planning activities such as family/patient education, nursing center documentation, and make referrals to appropriate agencies and suppliers.
• The Social Worker establishes relationships and maintain contact and referral flow with community based agencies/services

QUALIFICATIONS:

*Bachelor's degree from accredited school of Social Work or related field is required.

*At least one year of supervised social work experience in a health care setting, working directly with individuals, is preferred.

*Additional certification such as Geriatric Case Management, Hospice & Palliative Care, Gerontology, Clinical Social Work, Health Care, Nephrology, Mental Health, and/or Substance Abuse is preferred.
