Job Description

• The Social Worker collaborates with the interdisciplinary team to promote and protect resident rights and the well-being of all patients/residents. This includes educating staff on the role of the social services
• The Social Worker participates in the development of a written, interdisciplinary plan of care for each patient/resident to address social services needs
• The Social Worker hold meetings with patients/residents, families, and significant others as requested, to provide support and information to improve quality of life
• The Social Worker performs patient/resident discharge planning activities such as family/patient education, nursing center documentation, and make referrals to appropriate agencies and suppliers.
• The Social Worker establishes relationships and maintains contact and referral flow with community based agencies/services
