What does the Post-AcuteCare Specialist do?

ThePost-Acute Care Specialist acts as a liaison between hospitals and post-acutecare centers for smooth patient transfers to Prestige centers. Develops, implements,and maintains reporting systems and data analyses to drive growth of hospitalreferral program.

In this job, everyday you will:

  • Educatespatients on Prestige services and provides an efficient transition topost-acute centers. Coordinates with Prestige centers to ensure appropriate andtimely placement of patients.
  • Managespatient transfers to Prestige centers by coordinating physician orders, medicalequipment, medications and required documentation.
  • Evaluatespatient financial and insurance resources including, but not limited to,Medicaid, Medicare, HMO and private insurances and make recommendations forplacement for appropriate utilization.
  • Managesand tracks key program metrics for Prestige and hospital.
  • Collaborateswith Prestige centers to develop and maintain systems to evaluate a patient’srisk for hospital readmission and communicates recommendations on patientspecific interventions.
  • Partnerswith hospital case managers and Prestige centers to prevent unnecessaryreadmissions back to the acute setting. Reports on Return to Hospital (RTH)rate.
  • Createsand maintains reports on Centers for Medicare and Medicaid Services (CMS) andthe Center for Medicare and Medicaid Innovation (Innovation Center) paymentmodels.
  • Tracksreferrals and Prestige responses, admissions to Prestige from hospitals, andall denials by Prestige by discharge, length of stay, etc.

Education & experienceyou need to qualify:

  • Minimumof one year of experience in a health care environment with a working knowledgeof Medicare, Medicaid, Insurance funding systems required.
  • Bachelor’sDegree in business administration or equivalent knowledge and skills obtainedthrough a combination of education, training, and experience.
  • Mustbe detail-oriented and able to work in deadline driven situations.
  • Excellenttime management, organizational, and critical thinking skills.
  • Experiencein transition from hospital to skilled, assisted, or acute care settingspreferred

PTO:
SNF/AL/PCI Salary: Accrue PTO at rate of 0.0615/hour during their first year/16 days.
SNF/PCI Hourly: Full-time and Part-Time team members (over20 hours/week) accrue PTO at rate of 0.0423/hour during their first year.
