Position Summary

The Family Centered Service Coordinator (FCSC) coordinates mental health (MH) services and supports for child welfare-involved families when two or more members of the family are experiencing—or are at risk of experiencing—serious mental illness or serious emotional disturbance. The FCSC focuses on the needs of the entire family and coordinates a plan that addresses the individualized needs of specific family members and also the needs, goals and priorities of the family as a whole.

Essential Responsibilities

1. Develop relationships with all family members as applicable, as well as partners, other caregivers, and other important people as identified and with consent.
2. Work with the family to assess the family’s needs and strength.
3. Conduct risk assessments including suicide and violence risk assessments.
4. Collaborate with and support existing services and supports that may be working with a family, including individualized services.
5. Develop and implement proactive intervention strategies.
6. Work with the family to identify, develop, link, access, and coordinate natural and formal supports and resources including MH services and peer supports.
7. Participate in cross-system planning and teaming efforts with the family and other involved systems.
8. Identification and management of the symptoms of severe mental illness, co-morbid medical conditions, and dual diagnosis (mental illness/substance use or addiction/IDD).
9. Provides culturally competent services with consideration for the family’s racial, religious, sex, SOGIE, age and ethnic background and identification.
10. Maintain an accurate and timely record of activity, recording all family and collateral contacts.

Qualifications

• Clearances. State Police; FBI clearance; child abuse clearance; CPSL Mandated Reporter-Recognizing and Reporting Child Abuse training; any additional background checks/clearances required by state governing bodies.
• Valid driver’s license and current vehicle insurance.
