Job Title: Medical Care Coordinator
Overview: The Medical Care Coordinator plays a crucial role in the healthcare system by
facilitating communication and coordination between healthcare providers, patients, and
other stakeholders. This role involves managing the administrative aspects of patient care,
ensuring a smooth and efficient healthcare experience for assigned individuals. The Medical
Care Coordinator is responsible for organizing, implementing, and monitoring healthcare

Key Responsibilities:
1. Patient Liaison:
Serve as the primary point of contact for patients, addressing inquiries,
providing information, and offering support throughout the healthcare process.
Observe residents’ health status and take actions to address concerns within
scope of practice.
Facilitate effective communication between patients, healthcare providers, and
other relevant parties.
Monitor associates in the Supervision with self-administration of prescribed
mediation, including frequent audits of medication records.
2. Client Advocacy:
Advocate for clients to ensure they have access to appropriate shelter and
support services.
Work with community resources to address immediate needs such as food,
clothing, and medical care.
Respond to crises and emergencies, providing immediate support and
connecting clients with appropriate resources.
3. Care Plan Development:
Evaluate resident care needs and determine required level of care upon
admission and changes in condition.
Collaborate with healthcare professionals to create and update individualized
care plans based on patient needs, medical history, and treatment goals.
Ensure that care plans are comprehensive, realistic, and aligned with the
patient's overall health objectives.
4. Appointment Scheduling:
Coordinate and schedule medical appointments, tests, and procedures for
patients in collaboration with healthcare providers and medical facilities.
Monitor and manage appointment reminders to promote adherence to the care
plan.
5. Healthcare Resource Coordination:
Assist patients in accessing necessary healthcare resources, including
medications, medical equipment, and support services.
Collaborate with external agencies, healthcare providers, pharmacies, insurance
providers and community organizations to streamline access to additional
services and healthcare resources.
6. Documentation and Record Keeping:
Maintain accurate and up-to-date records of patient information, care plans,
and communication with healthcare providers.
Upload vital documentation into the electronic medical records as appropriate.
Ensure compliance with healthcare regulations and privacy standards in all
documentation.
7. Insurance Coordination:
Work with insurance companies to verify coverage, obtain authorizations, and
address any billing or claims issues.
Educate patients on their insurance coverage and assist in resolving insurance-
related concerns.
8. Patient Education:
Provide patients with information about their medical conditions, treatment
options, and self-care strategies.
Promote health literacy and empower patients to actively participate in their
healthcare.
9. Collaboration with Healthcare Team:
Work closely with case managers, social workers, and other internal
departments to ensure a coordinated and supportive environment for clients.
Foster strong communication and collaboration with physicians, nurses, social
workers, and other members of the healthcare team.
Participate in regular team meetings to discuss patient progress and address

Assist in the development and coordination of orientation of new organization
