Responsibilities:
- Patient Liaison for the residents and families
- Care Plan Development for residents by evaluating their current state and determining the action plan to healing.
- Assist residents in accessing necessary healthcare resources, including medications, medical equipment, and support services.
- Build relationships with local organizations, businesses, and government agencies to strengthen support networks.
- Engage in community outreach efforts to raise awareness of shelter services and resources.
Qualities:
- Knowledge of medical terminology
- BA in healthcare or nursing
- Experience with the homeless population
- Compassionate towards humans
- Experience in the medical field eg. EMT, Paramedic a plus
