Job Title: Admissions Coordinator
Overview: The Admissions Coordinator plays a key role in the Recuperative care setting, serving as a
liaison between prospective patients and the referring organization. This position involves guiding
individuals through the admissions process, ensuring all necessary documentation is completed, and
providing support to facilitate a smooth and positive entry into Recuperative Care.
Key Responsibilities:
1. Initial Contact and Inquiry Handling:
Respond to inquiries from referring facilities or prospective patients regarding admission
procedures, requirements, and programs.
Provide accurate and comprehensive information about the institution, its services, and
admission criteria.
2. Client Intake and Assessment:
Conduct initial assessments of individuals seeking shelter, gathering information on their
housing history, current situation, and support needs.
Determine eligibility for Recuperative services based on established criteria.
3. Application Processing:
Assist referring party in completing and submitting admission documentation.
Review referral documentation for completeness, ensuring all required documents and
information are provided.
4. Documentation and Record Keeping:
Maintain organized and up-to-date records of referral information, including demographics,
medical records, assessments, and other required documents.
Ensure compliance with privacy regulations in handling sensitive referral information.

Collaborate with operations department and other stakeholders to review applications and
assess candidates for admission.
Communicate admission decisions to referring parties and provide any necessary follow-up
information in a timely manner.
7. Interview Coordination:
Coordinate interviews with candidates as needed to verify appropriateness and willingness
to enroll into services.
8. Orientation and Onboarding:
Coordinate and facilitate orientation sessions for newly admitted individuals.
Provide information on the policies, procedures, and resources to ensure a smooth
transition for new patients.
9. Communication and Follow-up:
Maintain regular communication with applicants to provide updates on the status of their
applications.
Address inquiries and concerns promptly and professionally.
10. Client Advocacy:
Advocate for clients to ensure they have access to appropriate shelter and support services.
Work with community resources to address immediate needs such as food, clothing, and
medical care.
11. Collaboration with Other Departments:
Coordinate and facilitate the admissions process, including completing necessary paperwork
and documentation.
Collaborate with social workers, case managers, and other staff to ensure a holistic
understanding of each individual's situation.
Participate in cross-functional teams to address broader institutional goals.
12. Event Coordination:
Organize and participate in recruitment events, open houses, and informational sessions to
attract prospective partnerships.
Represent the institution at external events to promote admissions.
Qualifications:
Bachelor's degree in healthcare administration, nursing, or a related field.
Previous experience in admissions, homeless services, social services, or a related role is
beneficial.
Strong organizational, communication, and interpersonal skills.

Knowledge of community resources and support services for individuals experiencing
