Description of Responsibilities
The Licensed Practical/Vocational Nurse is responsible for providing direct patient care under the supervision of a registered nurse. Responsibilities include following the plan of care, providing treatments, and working collaboratively with the members of the team to help meet positive patient care outcomes.

Responsible for supervising home health aides.
Responsibilities include the following:
Provides direct patient care as defined in the state Nurse Practice Act.
Implements plan of care initiated by the registered nurse.
Provides accurate and timely documentation consistent with the plan of care.
Assesses and provides patient and family/caregiver education regarding self-care techniques and information pertinent to diagnosis and plan of care.
Participates in coordination of home health services, appropriately reporting the identified needs for other disciplines (HHA, OT, PT, MSW, ST, Dietician) to the registered nurse and/or Clinical Supervisor.
Uses and/or prepares equipment and supplies effectively and efficiently adhering to aseptic technique.
Participates in personal and professional growth and development.
Performs other duties as assigned by the registered nurse.

Participate in surveys conducted by authorized inspection agencies.
Participate in the Home Health’s Performance Improvement program as requested by the Performance Improvement Coordinator.
Participate in Home Health committees when requested.
Participate in in-service education programs provided by the Home Health Agency.
Report any misconduct, suspicious or unethical activities to the Compliance Officer.
Perform other duties as assigned by supervisor.
Minimum Qualifications:
Excellent observation, verbal and written communication skills, problem solving skills, basic math skills; nursing skills per competency checklist.
Prolonged or considerable walking or standing. Able to lift, position or transfer patients. Able to lift supplies and equipment. Considerable reaching, stooping, bending, kneeling or crouching. Visual acuity and hearing to perform required nursing skills.
Education and/or Experience:
1. Graduate of an accredited practical nurse or vocational nursing program.
2. Must have an IV/BW (Intravenous/Blood Withdrawal) Certification from a CA Board approved course provider.
3. Has minimum of two (2) years of nursing experience. With one (1) or more years’ experience in the Home health field.
4. Currently licensed as a licensed practical nurse or licensed vocational nurse in the state of California.
5. Possess and maintain valid CPR certification.
