Premier Home Health Care Services, Inc. is a leading established Home Health Care Agency. We currently have an opportunity for a Care Coordination Specialist in North Chicago, IL.

We are looking for a dynamic individual to provide support to our Care Managers scheduling assessments and providing ongoing communication with participants and updating the participant’s record.

Key Functions include:

• Schedule all assessments for participants and document all communication with participants.
• Call vendors to retrieve the proper contact information for participants.
• Be the main point of contact for the participants assigned and participate in the care planning process and development of Care Plans based on assessments received from the Care Manager.
• Data entry for all State systems as needed. Required to meet all timeframe measures.
• Assists in the coordination of services for assigned participants.
• Receive telephone calls related to participant’s safety incidents, provides all details to the Care Plan Specialists and assigned Supervisor.

Qualifications:

• Education: Minimum High School Diploma (or equivalent).
• Work Experience: Have at least one-year experience in health or human services.
• Travel: Travel may be required for business purposes. If so, the employee must have a valid driver’s license issued by the state in which they work and a satisfactory driving record.
• Skills

• Possess general knowledge of office procedures.
• Ability to use computerized systems, Word and Excel.
• Ability to follow instructions and work independently.
• Excellent customer service skills.
• Good verbal and written communication skills.
• Ability to work well under pressure.

Additional Requirements: Bi-lingual a plus
