Position : Patient Care Coordinator

Shift : Full Time 8 : 00 AM-4 : 30 PM M-F 80 HRS PER PAY

General Summary / Responsibilities :
The Infusion Center Patient Care Coordinator (PCC) is responsible for tasks that are integral to complex, interdisciplinary patient care coordination and is a multi-skilled individual able to perform administrative and clinical duties under the supervision of the Registered Nurse and delegated in accordance with the Ohio Board of Nursing.
The PCC is the initial point of contact for all patients and visitors to the department. This individual is responsible for administrative duties that may include coordinating all incoming calls, collecting a variety of patient data, entering patient information and charges into the hospital system, scheduling appointments, completion of registration by collecting and entering all pertinent financial and demographic information into the ADT system, verifying insurance benefit information, generation of the ABN, reviewing orders for compliancy, completion of MSP, obtaining financial and treatment consents, scheduling ordered medical tests and procedures, obtaining a pre-certification when applicable, preparation of patient hybrid charts, and providing clerical support while maintaining compliance with regulatory requirements.
Clinical duties may include assisting patients to and from the department, obtaining and
recording vital signs, assisting with patient care, identification of abnormal findings based on established standards and guidelines and reporting those findings to the Registered Nurse, and stocking / ordering of supplies.
The Infusion Center PCC must demonstrate customer focus with patience, composure, and compassion. Must be able to deal with ambiguity by effectively coping with change;
The Patient Care Coordinator is required to maintain excellent customer service standards at all times in order to effectively communicate with physicians, physician offices, patients, and co-workers.
Patient Care Coordinators are required to efficiently perform all duties while ensuring patient confidentiality and privacy rights.

Essential Duties & Functions :
1. Performs routine registration activities in an accurate and efficient manner, including scheduling appointment(s), validating all demographic, financial, and pertinent information necessary to meet all regulatory and billing requirements.
2. Hold financial clearance conversations with all patients. Including, but not limited to, verifying insurance eligibility and benefits, and calculating and collecting patient financial liabilities at the point of registration in a customer service-oriented fashion.
3. Utilization of required machinery for patient registrations (WOWs, eSignature pads, patient Kiosks, etc.).
4. Completion, documentation, and / or distribution of required forms (Annual demographics, ABNs, Financial Assistance applications, Privacy Notices, consents, etc.)
5. Provide excellent customer service while courteously answering telephone calls, responding to all inquiries, and effectively meeting customer needs.
6. Attends and completes required job-specific trainings and meetings as required.
7. Respond positively to change and demonstrate the ability to be flexible in the work environment.
8. Maintain an appropriate level of productivity and accuracy for work performed based on departmental standards, including pre-registration and daily completion of patient work queues
9. Performs and documents patient care skills completely and efficiently as delegated by the nurse upon verification of Support Associate competence.
10. Makes appropriate notifications regarding abnormal findings.
11. Promptly answers calls, responds to the needs of patients, visitors and / or family members and relays information / requests to appropriate person(s).
12. Appropriately secures forms and handles supplies / medications / lab specimens transported into / within unit.
13. Assists with the location and management of patient property.
14. Initiates maintenance requests orders, assures that maintenance requests for repair of defective equipment are completed promptly.
15. Prepares and maintains charts and associated documents. Assures that patient charts are accurate and updated.
16. Tracks and makes appropriate notifications regarding admissions, transfers, consultations, discharges, and deaths.
17. Consistently monitors the electronic medical record for any multi-step functionality orders that need to be completed with CPOM (computer physician order management) and carries out in a timely fashion.
Education : You'll need a high school diploma or GED.
Training : Before you start, you'll need to get a BLS CPR certificate . They can teach you how to do CPR at your orientation too, so you don't need to do it beforehand.
