Registered Nurse/ Licensed Practical Nurse

For over 35 years, Preferred Home Health Care & Nursing Services has been providing compassionate and dependable medical and non-medical home health care services throughout New Jersey, Pennsylvania, and Delaware. Through our quality care, we help our clients with the medical, physical, and emotional strain that comes when home health care is needed the most. Preferred is currently hiring a clinical supervisor for our Harrisburg, PA office.

Requirements:

· Valid RN License

· Physical within one year

· PPD or Chest X-Ray

· Valid Driver’s License

· Valid CPR certification

Responsibilities:

1. Provides daily supervision of nurses and home health aides to ensure quality of service is within the required plan of care

2. Initiates and reviews the clinical content, diagnoses, medical history, medications, and other statuses from client records

3. Appraises staff performance during the probationary period and annually with the director of nursing, and provides the teaching and counseling necessary to enhance the delivery of services and the professional development of nursing and home health aide staff

4. Identifies and evaluates client needs and appropriateness of home care by reviewing the initial intake screening form before assignment of direct care staff

5. Assists with the orientation of new professional staff to familiarize them with policies and procedures

6. Conducts client visits in the appropriate setting as needed

7. Initiates in-home supervisory visits of clinical staff as needed

8. Maintains confidentiality of medical records, personnel records, etc., within the scope of their position

9. Participates in any required functions in the absence of, or request by, the director of nursing

10. Works closely with the electronic medical record (EMR) to ensure client electronic charts are maintained, updated, and remain in compliance with company standards

11. Understands details around the knowledge of clinical workflow as it pertains to the use of EMR

12. Participates in formal and informal problem-solving processes to meet the EMR computer education needs and established outcomes for users

13. Provides trainer sessions for peers and field staff to expand training team expertise
