PRAIRIE VIEW is Hiring a Case Manager Near Newton, KS

POSITION GOAL: This position will help individuals experiencing or at risk of homelessness who have a serious mental illness, medical impairment, and/or co occurring substance use disorder apply for Social Security disability benefits, including Medicaid.

CPA 1: CLINICAL WORK *


  • Provide competent case management services in the community as assigned by supervisor.
  • Provide case management services based on the philosophies and principles of emphasizing patient’s strengths.
  • Assist in identifying, providing assertive outreach to, and establishing rapport with new case management patients.
  • Provide case management services using evidence based interventions to remediate symptoms of mental illness and/or improve emotional and functional well being.

Engage in the following patient centered case management activities as needed by each patient:

  • Assess strengths and set goals with the patient and when appropriate with involved others; review regularly.
  • Employ problem solving with patient and when appropriate with involved other to help them resolve issues and problems.
  • Provide appropriate crisis intervention assistance.
  • Assist the patient in getting linked to appropriate services based upon need.
  • Encourage patient to become more actively involved in the community.
  • Provide support and advocacy with various systems on behalf of patient to facilitate obtaining needed resources and /or protection of rights.

CPA 2: OTHER ADMINISTRATIVE RESPONSIBILITIES *


  • Assure clinical and administrative documentation policies are followed.
  • Assist in the completion of necessary state reporting on the status of patients.
  • Provide consultation and attend clinical staffings as requested.
  • Participate in clinical and administrative supervision with supervisor and peers.
  • Conduct meetings with staff who serve SOAR/SUPPORTED EMPLOYMENT individuals, for purpose of effective communication and program coordination.
  • Represent agency through attendance and participation in various groups and organizations on the local and statewide level.
  • Serve as extended member of the treatment team, backing up and assisting other community case managers as required.
  • Maintain responsibility for organizing a schedule and establishing priorities according to existing patient and agency needs.
  • Serve as SOAR representative for the purpose of applying for SOAR.
  • Conduct ongoing outreach and engagement with SOAR applicants.
  • Work cooperatively with SOAR state team.
  • Review completed Vocational/Educational Profile.

CPA 3: QUALITY ASSURANCE & PERFOMRANCE IMPROVEMENT (QAPI) *

  • Participate in organizational Quality Assurance and Performance Improvement activities. Be familiar with Quality Assurance and Performance Improvement initiatives and goals in work area. Annually review the Quality Assurance and Performance Improvement Plan.

CPA 4: PROFESSIONAL DEVELOPMENT *

  • Attend workshops, continuing education, and conferences in order to maintain and build skills and expand professional knowledge.
  • Actively read and explore innovative ways to provide services.
  • Use creative knowledge to expand and develop new program ideas, in collaboration with colleagues and supervisor.
  • Become a Certified SOAR Specialist for Kansas.
  • Maintain SOAR Specialist certification requirements.
  • Participate in continuing education activities and required inservices (such as Infection Control, etc.) as requested, to enhance and improve job performance.
  • Utilize Relias Learning site to enhance knowledge in the areas of mental health and treatment modalities.

POSITION DESCRIPTION

This position description is a temporary management guide tool and is subject to change.

POSITION QUALIFICATIONS:

  • Minimum Education: Bachelors degree in related field of behavioral sciences or equivalently qualified by work experience or a combination of work experience in the human services field and education
  • Minimum Experience: Experience working with individuals experiencing mental illness and/or homelessness population
  • Must be at least 18 years of age
  • Must demonstrate skills in the provision of case management
  • Must be able to complete required training within appropriate time frames
  • Must have good listening skills, verbal skills, and written communication skills
  • Must be able and willing to be flexible, a self starter, punctual, and have the ability to maintain a schedule
  • Must be able to relate to patients empathetically and non judgmentally
  • Must have a valid Kansas driver’s license and be insurable through Prairie View insurance company
  • Must be proficient at basic keyboarding/word processing skills
  • Must be able to pass Kansas Bureau of Investigation Screen and the Central Registry Screen for both adults and children
