Registered Nurse (RN) Case Manager

Position Duties and Responsibilities:

• Triage; patient assessment and monitoring; use of patient monitoring and treatment equipment; appropriate nursing care, procedures, and treatments; execution of physicians’ orders within the guidelines of standard nursing practice; documentation of patient care and observations; and patient education and emotional support.
• Participate in a multidisciplinary team with other health care providers providing consultative services pursuant to optimal health care delivery to patients suffering from pain. 
• Support patient self-management of patient care with external healthcare organizations and facilities, including hospital admission and discharge and referrals from primary care provider to specialty care providers and services in the medical neighborhood to include base resources. 
• Manage high risk patient care, including management of patients with multiple co-morbidities or high-risk readmission to a hospital setting and recurrent emergency department use to include use of registries and management of high utilizer, polypharmacy, and chronic pain patients.
• Assist patients with care coordination to include however, not limited to, assisting with referral issues, coordinating care with other clinical care areas within or external to the MTF and communicating with the patient via telephone and then documenting in an “in-between” encounter. 
• Participate in an on-site designated Extension for Community Healthcare Outcomes (ECHO) tele-mentoring training program.
• Participate in tele-mentoring, tele-consults, and tele-health to include care coordination/training.

Minimum Qualifications: 

• Bachelors of Science in Nursing.
• Graduate from a college or university accredited by National League for Nursing Accrediting Commission (NLNAC), or The Commission on Collegiate Nursing Education (CCNE).
• Certification: Possess one of the following certifications:
  • Commission for Case Manager Certification Certified Case Manager (CCM)
  • Certification of Disability Management Specialists Commission: Certified Disability Management Specialist (CDMS)
  • Association of Rehabilitation Nurses: Certified Rehabilitation Registered Nurse (CRRN)
  • American Board for Occupational Health Nurses Certified Occupational Health Nurse (COHN)
  • Certified Occupational Health Nurse-Specialist (COHN-S).
  • National Board for Certification in Continuity of Care: Advanced Certification in Continuity of Care (ACCC)
  • Commission on Rehabilitation Counselor Certification: Certified Rehabilitation Counselor (CRC)
  • American Nurses Credentialing Center Nurse Case Manager (RN-NCM)
  • National Academy of Certified Care Managers: Care Manager Certified (CMC)
• Possess three (3) years of experience in the last five (5) years as a RNCM.
• Current, full, active, and unrestricted license to practice as a Registered Nurse.
• Must be a US Citizen.
