Description

JOB SCOPE:

Provides oversight and monitoring of the referral and lease-up process for the Residential Division. Tracks all vacancies, obtains eligible referrals, and monitors the lease-up process across all housing programs. Tracks all admission and discharges to PCMH residential programs and completes all internal and external data reporting requirements to government partners.

ESSENTIAL FUNCTIONS:

  • Maintain knowledge of the eligibility criteria for all PCMH residential units.
  • Maintains general knowledge of supported and affordable housing operations within NYC.
  • Respond to all telephone inquiries regarding PCMH housing.
  • Maintain contact with Residential Program Directors and PCMH administrators to monitor and fill vacancies.
  • Serve as a liaison between HRA, HPD, SPOA, community service providers and PCMH programs.
  • Conduct phone outreach to hospitals and other community agencies to maintain a steady flow of referrals, as needed.
  • Schedule, monitor, and provide status updates on all intake referrals with both internal and external entities.
  • Serves as the primary point of contact with the HRA CAPS system: Maintaining apartment inventory, reporting on vacancies, requesting referrals, providing status updates, and completing all required reporting.
  • Serve as a System Administrator for the HRA CAPS website and provide support to other PCMH staff.
  • Track all admissions and discharge data in the CAIRS systems.
  • Schedule and conduct intake interviews with housing applicants for assessment at PCMH and at sites in the community, as needed. Conduct video-teleconference intake interviews with applicants, as needed.
  • Conduct criminal background checks for admissions to certain residential programs.
  •  Prepare written correspondence and notifications on applicant related issues.
  • Maintains productive working relationships with provider groups and referral sources.
  • Assists with LIHTC certifications, re-certifications, and compliance as needed.
  • Performs other duties, consistent with the goals and objectives of the Residential Division, or as assigned by the DVP Residential Operations.

Salary: $50,000 annually 

Schedule: Monday to Friday 9am-5pm

EDUCATION AND EXPERIENCE:

  • Minimum of a Bachelor’s Degree in a Mental Health discipline or related field is required.
  • Computer literate with knowledge of Microsoft Office, spreadsheets, and publishing software.
