PMHCC, INC. is Hiring a Social Drivers Specialist Near Philadelphia, PA

Position Summary:

The Social Drivers Specialist reports to the DBHIDS System’s Integration, SDOH Equity Unit, Manager of Social Drivers and Clinical Collaboration. This position coordinates and manages programs that address that address SDOH such as employment, transportation, technology and food to support participants in improving clinical outcomes. The role will include cross system stakeholder engagement to enhance outcomes, coordinating and facilitating work groups and meetings, programs, and contracts. This position is responsible for supporting individuals receiving services from SDOH domains with assessing and then navigating physical and behavioral healthcare providers to complete referrals to improve individual care and outcomes.

Duties and Responsibilities:

  • Implement, coordinate, and manage various projects and activities as assigned
  • Plan, coordinate and facilitate meetings with internal and external stakeholders.
  • Centralized DBHIDs Social Drivers Specialist projects
  • Establish and expand linkages with social determinant resources to bring services to our participants.
  • Work cross departmentally to receive referrals and connect participants to programming. Enter track the deployment of resources to participants. Enter work with departmental teams to support clients.
  • Work with clients to ensure coordination of resources as needed.
  • Liaison cross departmentally and cross agency as needed, to execute programs.

Skills Required:

  • Excellent oral and written communication skills
  • Project management experience
  • Ability to work independently and as part of a collaborative clinical team as the situation requires.
  • Excellent interpersonal, written, and verbal communication skills.
  • Organized, flexible and ability to multitask.
  • Knowledge of computer software applications (Microsoft Office Applications, Excel others)
  • Knowledge of grammar, spelling, and punctuation as it relates to correspondence necessary to perform essential job functions.
  • Knowledge of basic arithmetic to make calculations, balance and reconcile figures necessary, and make changes accurately.
  • Excellent customer service skills

Education and Experience:

  • Bachelor’s degree in health and human services field
  • Experience working with community members, individuals receiving services across DBHIDS and stakeholders collaboratively.
  • Experience supporting or managing workgroups and committees.
  • Experience and in-depth experience with one or more of the following:
  • Food insecurity – understanding the landscape of low-income food resources and providers. Participation and management in work groups to address strategies to deploy food services to large groups of individuals and or health households.
  • Transportation resources – work with SEPTA and/or Uber health and/or Lyft and/or Catch, to support people connecting to behavioral health resources and opportunities to increase their economic mobility.
  • Technology – understanding of resources for low-income persons to connect with physical equipment and low-cost Wi-Fi services.
  • Economic Mobility-manage the Workforce Referral Form as well as provide technical assistance to DBHIDS employment providers.
  • Previous work with agencies that provide food, transportation, or technology resources to participants.

Physical Demands:

  • Ability to travel up to 50% of the time across the Philadelphia community, in individual’s homes or healthcare settings.

Essential Functions:

  • Manage the SDOH Supports Program Data Clearinghouse.
  • Coordinate distribution of SDOH Supports Program resources amongst stakeholders and community members.
  • Manage and implement OHS Prevention Program for adults and youth in emergency housing sites.
  • Manage the data input into the Executive Management Dashboard from identified SDOH Projects
  • Maintain partnerships amongst community based organizations, attend taskforce meetings as assigned
  • Co-facilitate monthly DBHIDS SDOH Workgroup Meetings
  • Explore additional funding/philanthropic opportunities to sustain the SDOH Supports Program.
  • Special projects related to addressing the social determinants of health as needed.
  • Special projects related to clinical supports within the department and with partner agencies and organizations as needed.
