ADMISSIONS LIAISON

The Admissions Liaison must have a current license as a LPN or RN in the state of PA. Is primarily based in the hospital with at least one day per week in the facility to process referrals for the nursing home to assess patients, review their medical records, and determine whether they are candidates for admission to the facility for care and services. This position requires excellent clinical assessment skills as well as knowledge of Medicare and Medicaid Case-Mix, pharmacy costs, and verification of health insurance and its coverages. The liaison will complete a clinical and financial assessment and determine suitability for admission based on the medical and financial information. The liaison must be knowledgeable of State and Federal Regulations for Long Term Care. The liaison must possess excellent interpersonal skills and be able to represent the facility in a positive manner to patients, families, hospital personnel and the general public. The Liaison further works to develop new business relationships within the community, attends or host communed community events, as directed, and maintains existing relationships with community referral sources, physicians, agencies, etc. Works collaboratively with the Regional VP of Business Development and VP of Physician and Community Services.

ESSENTIAL REQUIREMENTS, DUTIES, AND RESPONSIBILITIES

1. Early identification of candidates/potential admissions to the facility from all referral sources.

2. Screen potential patients through on site and/or telephonic assessment and EHR record review in collaboration with physicians, nurses, therapists, social workers, case managers and other referral sources.

3. Respond promptly to all referrals with the intention to admit

4. Gains approval to deny admission from Executive Director or Administrator

5. The liaison will closely interface with the Omega Bed Social Worker at York Hospital to identify and screen potential placements as well as with other inter-disciplinary team members involved in patient’s care.

6. Determine the client's rehabilitation potential, discharge plan and feasibility, and specialized rehabilitative or equipment needs.

7. Complete all required preadmission screening documentation to determine that services are appropriate, reasonable and necessary.

8. Assess referrals and make admission decisions based on medical and clinical needs, financial information and in accordance with State and Federal Regulations, and in coordination with facility staff as indicated.

9. Communicate to the facilities admissions department and other pertinent staff all information necessary for admission. Provide recommendations for appropriate level of care.

10. Interact with potential resident and/or family to complete the admission application.

11. Provide needed hospital information for new admissions for MDS completion.

12. Participate in monthly care plan review conference for Omega Bed patients.

13. Assist Omega Bed Social Worker with tracking of all patients transitioned to Omega Bed for duration of stay and provide in-person transition support to promote continuity of care.

14. Educate patients, families and staff regarding the facility and Premier, the facility record of providing quality care, and promote effective public relations with clients and families, healthcare providers, and the community.

15. Exhibit professional behavior as a representative of the facility and Premier.

16. Must have a current license in good standings to practice as an LPN or RN in the State of Pennsylvania.

17. Other reasonable duties as assigned by supervisor.

Requirements are representative of minimum knowledge, skills and abilities. To perform this job successfully, the incumbent will possess the ability or aptitude to perform each duty proficiently, and comply with Association policies and procedures, as outlined in the Employee Handbook.

ESSENTIAL PHYSICAL AND MENTAL JOB REQUIREMENTS

Must be proficient in reading and writing and understand the spoken and written word. Must be able to apply good sound nursing judgement while applying safety measures and regulations. Must have some computer skills. Must be able to hear with corrective or adaptive devices and see 20/20 with corrective eyewear. Must have simple computational skills such as addition, subtractions, multiplication, division and percentages. Must be able to stand for long periods of time, bend at knees and waist, push and pull, turn and lift a minimum of 50lbs
