• Patient Support: Patient care coordinators serve as a point of contact for patients and their families, offering support and guidance throughout their healthcare experience. They may assist with appointment scheduling, answering inquiries, and providing information about medical procedures or treatment plans.
• Care Coordination: Coordinating patient care involves working closely with various healthcare professionals, such as doctors, nurses, therapists, and social workers. Patient care coordinators ensure seamless communication and collaboration among the care team members to develop and implement comprehensive care plans tailored to the individual patient's needs.
• Patient Advocacy: Patient care coordinators advocate for patients' rights and ensure that their preferences and concerns are addressed. They act as a liaison between patients and healthcare providers, helping to resolve any issues or conflicts that may arise during the course of treatment.

We are looking for a Patient Care Coordinator to join our team and provide excellent patient care. The Patient Care Coordinator will be responsible for scheduling appointments, coordinating patient care plans, and providing administrative support to the medical staff. Responsibilities:

• Ensure that patient care is provided in a timely and efficient manner.
• Maintain accurate patient records and ensure that all documentation is up to date.
• Provide support to patients and families during hospitalization and after discharge.
• Coordinate with physicians, nurses, and other healthcare providers to ensure quality care for patients.
• Provide patient education on health topics, medications, and treatment plans.
• Assist with scheduling appointments and tests.
• Monitor patient progress and report any changes to the healthcare team.

Pay: $20.00 - $23.00 per hour
