*If you qualify for this position and are interested, please put a note in your application with the following information:*

1. This is a full-time position. Are you willing and able to work full-time (Monday through Friday 8am - 5pm)?

2. What times and days could you do an in-person interview?

3. Before interviewing, the manager is requesting to know your specific salary range expectation?

4. Please tell me why you are interested in working in the hospice setting?

5. Do you have current and valid CPR certification, Driver's License, and auto insurance?

6. If you saw a pt exhibiting a new symptom, would you be more likely to ask the MD to add a med that would handle the new problem or to look to see if that is a side effect of any of the drugs they are on and request the MD to reduce the drugs that might be causing that as a side effect?
