Demonstrates and promotes participative leadership and provides oversight of the nursing department by building relationships, acting with integrity and humility, modeling the way, listening with the intent to be influenced and embracing change.
The Resident Assessment Coordinator is responsible for the following duties and responsibilities which are considered essential functions of the position.
vCultivates relationships with residents, families, staff, and visitors, fosters a vibrant work environment/culture and promotes Pine Acres Rehabilitation & Care Center’s mission, vision, values and goals.
vImplements established performance standard of nursing practice and facility policies and procedures.
vProvides direct resident care as necessary, performs nursing skills, demonstrating knowledge and competence.
vGreets resident by name and listens to resident, family and physician concerns attentively and initiates corrective measures and/or reports concerns to the Director of Nursing and/or Administrator.
vOversees and completes the MDS for all residents according to the standards set by CMS.
vOversees and completes the resident care plan according to the federal rules and regulations.
vConducts resident care conference with facility personnel providing direct care.
vOversees and completes Quality Assurance and Performance Improvement (QAPI) audits, quality measure compliance and ensures excellence in quality care.
vConducts counseling/education sessions with nursing personnel as delegated by the Director of Nursing. Reports outcomes of all counseling/education sessions to the Director of Nursing. Initiates corrective action as deemed necessary. Reports concerns identified and corrective measures to the Director of Nursing and/or Administrator.
vDemonstrates consistent ability to work cooperatively with facility Administrator, Director of Nursing, nursing personnel, physicians, residents, families, other facility personnel, consultants, community agencies, and ancillary service providers.
vMaintain resident information and internal affairs in strict confidence.
vSustains the appearance of nursing areas; regularly inspects units for sanitation, order, safety and proper working condition of equipment.
vPerforms duties of Staff Nurse whenever and wherever required.
vAccepts on-call for emergencies that the Charge Nurses and/or Nurse Managers cannot resolve.
vAssures proper handling and emergency care of patients, personnel and visitors involved in accidents while on the job or in the building.
vKeeps current in the field of nursing through attendance to meetings, conferences, conventions and workshops pertaining to all aspects of patient care and rehabilitation.
vOther job duties assigned as deemed necessary by the Director of Nursing and/or Administrator.
vGeneral Qualifications
oGood physical and mental health.
oSound judgment and high moral standards.
oSincere desire to work with the aged and those with limited capacity for self-care.
oAbility to organize and plan nursing service department.
oMust be well-groomed, courteous, tactful, patient, kind and pleasant when dealing with patients.
oMust be level-headed in emergency situations.
oMust be self-motivated and have an acute sense of responsibility.
oMust possess a spirit of cooperation and enthusiasm in order to create an atmosphere conducive to rehabilitation and growth.
vEducation
oGraduate of an approved school of nursing.
oRegistered nurse or ability and willingness to attain preferred.
oMust have current licensure in the State of Iowa or the ability to attain licensure.
vContinuous Education
oKeeps current in the field of nursing through attendance at meetings, conferences, conventions, and workshops; reading current literature pertaining to all aspects of patient care and restoration/rehabilitation; maintaining membership in professional organization.
vWork Experience
oMinimum of 2-years supervisory experience in a hospital or post-acute care facility, including geriatric and/or psychiatric nursing.
oTraining in rehabilitative and restorative nursing care.
oMinimum of 2-years MDS and care plan experience is preferred by not required.
vLifting Requirements
oThe Nurse Manager may frequently be called upon to assist with transfer of residents and lifting of equipment and supplies.
vCarrying Requirements
oThe Nurse Manager is regularly called upon to carry objects including charts and equipment. The Nurse Manager occasionally will be called upon to carry equipment and supplies.
vSight Requirements
oThe Nurse Manager is required to have 20/20 correct vision.
vHearing Requirements
oThe Nurse Manager is required to have normal range of hearing as corrected.
vOther Physical Requirements
Working Conditions:
vMust communicate with medical staff, other department heads, nursing personnel, and patients and their relatives.
vSubject to frequent interruptions and called upon frequently to handle patient issues.
