The primary responsibility of the Social Worker is to provide social work care management services to the participants of the Piedmont Health SeniorCare program; provides services such as participant screening, assessments, case/and or care management, supportive listening, and coaching and referrals. Requires frequent sitting for long periods, the operation of standard office machines and computer. May require lifting of up to 25 pounds. Requires eye-hand coordination and manual dexterity. Requires the use of office equipment, such as computer terminals, telephones, and copiers. Requires normal vision. Work is performed in a Day Center environment.
