Partnership Heath part of the Integrity Family is seeking Part Time LPN in our Toms River office.

Our Toms river office is open only to Teachers and their families working for the Toms River School District. This office offers onsite Family Practice, X-ray, Lab, Pharmacy, Physical Therapy and Behavorial Health

This position will be working directly with our Care Coordinators managing our high risk and CC cohorts needing attention, calls to members, research and followup on specialists, labs, and yearly Physical Examinations

Schedule

24 hours per week 3 day- 8 hr shifts during the week - 8-4 or 9-5 (may also consider 2 days)

Prior care coordination or case management experience preferred.
