About us One Twelve is a 245 G Licenced ASAM 3.1 SUD treatment facility in Minnetonka, MN. We are a 12 bed residential facility that serves men 18 . The core of our program is the 12 Step fundementals. Since 2014 we have strived to give men a chance going from inpatient to outpatient settings.
Responsibility: To provide 245G and 1115 Waiver compliant clinical services to adult men in a residential setting. Services include assessments, assisting in program intakes, group therapy, individual therapy, individualized treatment plans, treatment coordination and assistance in the continuum of care planning. Maintain licensure compliant documentation. Provide client centered services to ensure clinical service compliance, outcomes, and client safety.
JOB DUTIES:
Implement clinical services as directed by the Programs Director and required by funders and agency business plan
Comply with clinical service policies and relevant agency policies
Provide clinical services that are client centered, cost effective, and meet all goals/objectives within licensed required reporting periods
Follow all systems in place for the safety and welfare of clinical service clients
EDUCATION AND EXPERIENCE:
Current license to practice Alcohol and Drug Abuse Counseling in a 245G licensed facility or is exempt from licensure under MN Statute 148C
Mental Health professional experience
Minimum of 1-year experience in the delivery of clinical services in a 245G setting
High degree of computer literacy including experience using an electronic health records (EHR) system, preferably Procentive.
Knowledge of 12 Step self-help groups.

Demonstrate knowledge of evidence based clinical service tools in a substance abuse model of service delivery
Demonstrate a knowledge of working with a population with co-occurring issues
Demonstrate knowledge of working with a diverse population
Demonstrate knowledge of trauma informed delivery of services
Demonstrate the practice of high ethical standards
Must be able to pass a MN DHS background study.
Must be free of problematic substance use for at least two years immediately preceding employment and must sign a statement attesting to that fact
