Phoenix Health Services is Hiring a Certified Substance Abuse Counselor Near Baltimore, MD

***Requires a valid Maryland ADT, CAC or CSC-AD***

Hours: Flexible; Full-time

Please confirm that you have the following experience:

-Professional 2 yrs minimum working in true residential setting 3.1, 3.3, 3.5, 3.7 (Not transitional housing)
-Extremely proficient with computers and technology.
-Experience doing residential groups and being creative with groups.
-Proficient with completing groups notes and individual client-centered notes.

Under the supervision of the Clinical Supervisor, the Addictions Counselor will work in a multidisciplinary team environment, providing frontline care to consumers in the Intensive Outpatient Program, Outpatient Program or Residential Treatment programs. This position is responsible for assisting clients in the development and maintenance of healthy lifestyles and recovery management skills, with a focus on addiction and substance abuse. The Addictions Counselor will utilize their expertise in addiction treatment, substance abuse counseling, and dual diagnosis to support clients on their recovery journey.

Essential Duties:

  • Maintains a caseload as assigned, focusing on addiction and substance abuse treatment, in accordance with COMAR and Joint Commission regulations.
  • Ensures incoming clients have a complete psycho-social history and comprehensive assessment and interpretative summary within 5 days of admission, with a specific emphasis on addiction and substance abuse.
  • Prepares, with client participation, a Comprehensive Treatment Plan for all persons served within 30 days of admission, addressing their addiction and substance abuse issues.
  • Completes a Treatment Plan Update every 30, 60, or 90 days according to regulations, specifically addressing the progress and challenges related to addiction and substance abuse treatment.
  • Performs individual counseling with clients on a regular basis, focusing on addiction and substance abuse issues, as well as emergency intervention when necessary.
  • Performs Group counseling on a rotating basis as assigned by Clinical Director/Program Administrative Director, with a focus on addiction and substance abuse topics.
  • Documents client progress and all interventions in the client record via EMR, ensuring accurate and detailed information related to addiction and substance abuse treatment.
  • Prepares an aftercare plan for all persons served completing the program or being discharged for other reasons, emphasizing relapse prevention and ongoing addiction support.
  • Completes discharge summaries for assigned caseload within the time allotted per State regulations, highlighting addiction and substance abuse treatment outcomes and recommendations.
  • Provides follow-up care and contact with each discharged consumer, offering ongoing support and resources related to addiction and substance abuse recovery.
  • Provides coordination of care of health, housing, and community services according to the Treatment Plan and individual need, particularly as they pertain to addiction and substance abuse support.
  • Ensure the completion of client caseloads, treatment plans, progress notes, discharge summaries, and other essential documentation in accordance with applicable federal and state standards as well as agency policies and procedures, with a specific focus on addiction and substance abuse treatment.
  • Keep all client files updated in accordance with established policies and procedures, emphasizing accurate and up-to-date information regarding addiction and substance abuse treatment.
  • Counsel patients in crisis situations by identifying patient resources and coping abilities to help them cope with and take steps to resolve addiction and substance abuse-related crises.
  • Arrange referrals to other agencies and resources in the community when appropriate, specifically related to addiction and substance abuse support services.
  • Perform treatment plan reviews in a timely manner as stated by state and agency regulations, ensuring ongoing evaluation and adjustments in addiction and substance abuse treatment plans.

This position requires a thorough understanding of the organization's philosophy of treatment, admission process, discharge criteria, and aftercare services in the context of addiction and substance abuse. The Addictions Counselor will apply their basic knowledge of substance abuse and dual diagnosis to ensure that clinical and administrative services are provided to all consumers in accordance with state regulations and accreditation standards related to addiction treatment.
