We are seeking a dedicated and compassionate Case Manager to join our clients team. As a Case Manager, you will be responsible for providing support and guidance to individuals and families in need.

Temp to Hire

• Coordinate and monitor services to ensure the delivery of appropriate and timely support
• Conduct comprehensive assessments of 25 clients and their families
• Provide crisis intervention and manage emergency situations as needed
• Collaborate with healthcare professionals, social workers, and community organizations to facilitate access to necessary resources
• Advocate for clients' rights and ensure they receive the appropriate services and benefits
• Responsible, organized, effective, and efficient. Ability to perform multiple assignments simultaneously.
• Skills:
• Bachelor's degree in social work or related field or Associates Degree with 2 plus years of experience-
• Proven experience in case management or a related field-
• Strong crisis management skills with the ability to remain calm under pressure
• Excellent communication skills, both verbal and written .
