The RN Case Manager, in collaboration with the clinical team, provides discharge planning coordination and intervention. The RN Case Manager will perform concurrent and retrospective review of medical records for purposes of utilization management and compliance with requirements of external review agencies including governmental and non-governmental payers. The RN Case Manager strives to promote patient wellness, improved patient outcomes, efficient utilization of health services and minimize denials of payment.
Required Education:
Graduation from an accredited School of Nursing
Preferred Education:
Bachelor of Science in Nursing
Required Technical Skills:
Basic computer and keyboarding
Preferred Technical Skills:
Meditech Electronic Medical Record
Required Licensure/Registration/Certification:
Registered Nurse, Licensed in the state of Texas or Compact State
Preferred Licensure/Registration/Certification:
Certification in Case Management
Required Experience:
Three years acute care nursing experience
Preferred Experience:
Case Management experience
