The LVN Case Manager, in collaboration with the RN-Case Manager and clinical team, provides discharge planning coordination and intervention. The LVN Case Manager strives to promote patient wellness, improved patient outcomes, and efficient utilization of health services. Works together with the patient and family, care team, payers, and external entities to promote a safe transition from the hospital to the next care setting.

The LVN partners with the RN Case Manager to execute the discharge plan and coordination of care for next site of care. Stays compliant with regulatory requirements regarding discharge planning and utilization management (ex. Medicare notice letters). Communicates any problems or issues that affect patient flow, patient satisfaction, patients safety, length of stay management, or outcomes to the department director or key stakeholders. Stays abreast of changing organizational, clinical trends, regulatory matters and third party requirements related to clinical care, discharge planning, and precertification or aftercare benefits.

This position works varied days/shifts.

Required Education:
Graduation from an accredited School of Nursing
Required Technical Skills:
Basic computer and keyboarding
Preferred Technical Skills:
Meditech Electronic Medical Record
Required Licensure/Registration/Certification:
Active and unrestricted LVN Texas License
Preferred Licensure/Registration/Certification:
Certification in Case Management
Required Experience:
1 year experience in a clinic setting
Preferred Experience:
Case Management experience
