Overview:
We are seeking a dedicated and (CERTIFIED) compassionate Case Manager to join our team. As a Case Manager, you will play a vital role in coordinating and advocating for the needs of our patients. Your primary responsibility will be to ensure the delivery of high-quality patient care and facilitate successful patient outcomes. Responsibilities:
- Collaborate with healthcare providers to develop individualized care plans
- Coordinate and monitor patient progress throughout their healthcare journey
- Provide education and support to patients regarding treatment options and resources
- Assist with discharge planning and ensure appropriate follow-up care
- Utilize medical terminology and knowledge of diagnostic evaluations to effectively communicate with healthcare professionals
- Adhere to HIPAA regulations and maintain patient confidentiality
- Participate in utilization management activities to optimize patient outcomes and resource utilization
- Collaborate with interdisciplinary teams to ensure seamless coordination of care
- Advocate for patients' rights and facilitate access to necessary services
Experience:
- Bachelor's degree in Social Work or related field required; Master's degree preferred
- Previous experience in a hospital or healthcare setting strongly preferred
- Knowledge of behavioral health principles and practices
- State Certification (current and up to date) required
- Familiarity with discharge planning processes and community resources
- Strong communication skills, both written and verbal
- Ability to work independently as well as part of a team
- Excellent organizational skills and attention to detail
