Position Description:
LEAD Case Management
Lead exists to establish a new system of response and care for people who live with unmanaged behavioral health needs, deep expierences of complex trauma, cognitive disabilities, persistent poverty and often lifelong expierences of punishment, failure, betrayal, and marginalization- people who are not served by office-based, appointment- based, and time-limited care.
Minimum qualifications of Case Manager
Bachelor's degree in human services or related field with a minimum of two years' expierence. Knowledge of harm reduction approaches ad a willingness to work outside of a office setting with individuals with complex needs. Posses a driver's license with the ability to be insured. Undergo and pass background checks and clearances prior to starting in their position.
Duties of the Case Manager
Manage participant case load of no more than 25:1 ; given LEAD participant's' complex and long-term needs by assessing their individuals needs and problem solving with participants
Provide case management within the community ( street-based) to meet the client where they are at physically and figuratively
Help particpants identify wanted and needed services and register/connect them for those services
Connects particpants to all avilable resources and help them schedule appointments as needed
Transport particpants to services and provide support at those service appointments via agency vechile
Attends all policy coordinating group (PCG), operations working group (OWG), and all other meetings assigned by supervisor or delegate and provides updates and reviews regarding caseload particpants and advocates for particpants' additional needs.
Must develop and maintain positive, collaborative relationships with all LEAD partners and other service providers, law enforcement, and LEAD participants demonstrating ability to provide street-based outreach and engagement with complex population.
Work as a street-based team with case managers for connecting participants face to face
Wiling and able to engage in policies and procedures related to managing crisis situations' in the community as outlined by the Dutchess County Department of Behavorial and Community health and the LEAD project manager. ( May include but not limited to contacting Helpline, Mobile crisis team, the police, and/or utilizing the Dutchess County Stabilization Center)
