JOB
The primary purpose of this position is to provide public health nursing services to the citizens in Pender County by promoting good health care through early detection and intervention, primary prevention, community awareness and education. This Public Health Nurse will serve as the coordinator for the BCCCP and Lead programs and provide back-up to Child Health/Adult Health, Women’s Health, and General ClinicTo perform this job successfully, an individual must be able to complete the essential functions of the position with or without reasonable accommodations. EXAMPLE OF DUTIES
Serve as Program Coordinator for BCCCP. Review and update program policy and procedures manual annually and as needed with assistance from the Nursing Supervisor.Coordinate with Management Support Staff regarding clinic appointments for patients receiving BCCCP services.Interview/screen all BCCCP patients presenting for services and document all patients' records and forms according to the requirement of each.Ensure follow-up of all abnormal lab and radiology results per provider's instructions.Promote the BCCCP program by providing outreach education to the community through brochures, flyers, media releases and community presentations to private physicians, church groups and other organizations.Enter all patient information into the electronic health record; approve all BCCCP radiology billings for payment.Attend required trainings for the BCCCP Program.Serve as back-up nurse for the Child Health/ Adult Health clinicServe as Coordinator for the Lead program. Review and update program policy and procedures manual annually and as needed with assistance from the Nursing Supervisor.Review lead levels of children screened and follow up on elevated lead levels with education and referral to appropriate agencies and services (WIC, Environmental Health).Triage patients using nursing judgment as to severity of patient's condition and the need for immediate attention.Perform screening, interviewing and counseling as part of patient education and intervention.Accurately complete histories and other required program documentation.Administer vaccines and report any adverse reactions related to immunizations. Document immunizations in the North Carolina Immunization Registry (NCIR).Assist the FNP as needed.Review patients' electronic health records after each clinic for accuracy and completeness of necessary data, treatment, referral, and follow up.Coordinate referrals, make appointments, and communicate with Primary Care Nurse to ensure follow up.Coordinate referrals as ordered and ensure follow-up.Follow up on all abnormal lab results per provider's instructions.Dispense or administer medications and vitamins from the pharmacy per written orders or standing orders. Provide appropriate counseling and instructions for follow up.Serve as back-up nurse for General Clinic and Immunizations.Administer vaccines to walk-in and follow-up children and adults seen in General Clinic and report any adverse reactions related to immunizations. Document immunizations in the North Carolina Immunization Registry (NCIR).Dispense or administer contraceptive medication, multi-vitamins or other medication from the pharmacy per written orders or standing orders. Provide appropriate counseling and instructions for follow up.Administer and read TB skin tests.Complete the TB Epidemiology form for all positive TB skin tests when the Communicable Disease Nurse is not available.Give urine pregnancy test results. Provide appropriate education based on test results per standing order.Administer various injections, such as Haldol, and allergy injections to patients with a physician's order.Triage patients when Nursing Supervisor or DON are unavailable and use nursing judgment as to severity of patient's condition and the need for immediate attention.Accurately complete all required documentation in patients' electronic health record.Serve as back-up to the Family Planning Nurse.Interview family planning patients for all required elements including medical history, partner history, reproductive history and psychosocial history according to established criteria. Accurately complete all required documentation.Dispense or administer contraceptive medication, multi-vitamins or other medication from the pharmacy per written orders or standing orders. Provide appropriate counseling and instructions for follow up.Provide reinforcement for proper use of chosen method of contraception and instructions for emergencies and follow up services.Make referrals as ordered and ensure follow up.Review patient records after each visit for accuracy and completeness of necessary data, treatment, referral, and follow up.Follow up on all abnormal lab results per provider's instructions.Assist in colposcopy clinic as needed by the provider WHNP/physician.Serve as back-up to the Maternal Health Nurse.Interview prenatal patients for all required elements including medical history, obstetrical history and identification of high-risk factors according to established criteria. Accurately complete all required documentation.Follow up on all abnormal lab results per provider's instructions.Review patients' electronic health records after each visit for accuracy and completeness of necessary data, treatment, referral, and follow up.Dispense or administer prenatal vitamins and medications from the pharmacy per written orders or standing orders. Provide appropriate counseling and instructions for follow up.Interview STD patients for all required elements including medical history, STD risk assessment and social history according to established criteria. Accurately complete all required patient documentation in patients' electronic health record.Administer vaccines as needed and report any adverse reactions related to immunizations. Document immunizations in the North Carolina Immunization Registry (NCIR).Follow up on all abnormal lab results per provider's instructions.Dispense or administer medication from the pharmacy per written orders or standing orders. Provide appropriate counseling and instructions for follow up.Review patients' electronic health records after each visit for accuracy and completeness of necessary data, treatment, referral, and follow up.Perform other duties as assigned by the Nursing Supervisor/DON as necessary to provide effective public health services.Participate in medical record audit reviews, staffing disaster shelters, educational in-service programs, professional development and training exercises.Make Nursing Supervisor/DON aware of any problems in clinic management and offer recommendations.Other related duties as assigned.
SUPPLEMENTAL INFORMATION
This work requires the occasional exertion of up to 10 pounds of force; work regularly requires speaking or hearing, using hands to finger, handle or feel and reaching with hands and arms, frequently requires standing and occasionally requires walking, sitting, pushing or pulling and lifting; work has standard vision requirements; vocal communication is required for expressing or exchanging ideas by means of the spoken word; hearing is required to perceive information at normal spoken word levels; work requires preparing and analyzing written or computer data, operating motor vehicles or equipment and observing general surroundings and activities; work has no exposure to environmental conditions; work is generally in a moderately noisy location (e.g. business office, light traffic).
