Job Overview:
We are seeking a highly organized and compassionate individual to join our team as a Service Coordinator. As a Service Coordinator, you will be responsible for managing and coordinating services for individuals with developmental disabilities. This is a rewarding role that requires strong interpersonal skills, and a commitment to improving the lives of others.

Duties:
- Develop and implement individualized service plans for clients with developmental disabilities
- Coordinate and monitor services provided by various agencies and service providers
- Conduct assessments to determine client needs and develop appropriate goals
- Collaborate with clients, families, and caregivers to ensure services meet their needs
- Provide ongoing support and advocacy for clients to ensure their well-being and access to necessary resources
- Monitor client progress and make adjustments to service plans as needed
- Maintain accurate documentation of client information, services provided, and progress made
- Collaborate with other professionals such as social workers, behavioral health specialists, and medical professionals to coordinate comprehensive care

Skills:
- Strong organizational skills with the ability to manage multiple tasks and priorities effectively
- Knowledge of working with individuals with developmental disabilities
- Understanding of behavioral health principles and strategies
- Excellent relationship skills to collaborate with clients, families, caregivers, and service providers
- Strong administrative skills for documentation and record keeping

MUST HAVE A BACHELOR'S DEGREE IN HUMAN SERVICES, PSYCHOLOGY, SOCIAL WORK, EDUCATION, SOCIOLOGY OR RELATED FIELD.
