Job Summary

We have an immediate opening for an experienced APRN to provide services in Long term care facilities. The successful candidate will be a highly motivated member of the team providing excellent care based on medical necessity to our patients. This candidate will be a self-starter, with excellent attention to detail, able to complete their work in the most efficient manner. This position offers competitive salary and benefits.

Responsibilities and Duties

• See scheduled patients in Long Term Care facilities as needed.
• Will complete work/documentation within 48 hours of visit.

Qualifications and Skills

• Must have an unrestricted license in Kentucky. Willing to obtain Indiana license.
• Must have 2-3 years’ experience in clinic or Long Term Care facility.
