How you will make an Impact:

Provide quality patient care that is consistent with the mission of PCC organization. Provide leadership support for the clinical staff.

Essential Duties and Responsibilities:

  • Obtains vital signs and performs routine functions to prepare patients for examinations or procedures.
  • Assists providers with treatments and procedures as directed.
  • Assesses the patients condition and utilizes teach back method to provide necessary patient education.
  • Assesses walk-in patients as well as other RN driven visits.
  • Performs and documents basic patient history, treatments, and administered medications appropriately in EMR.
  • Administers prescribed medications and treatments as directed by the provider following PCC policies and procedures.
  • Accurately documents in EMR and reports to the provider any pertinent information regarding patients condition and care.
  • Reviews patients records and other pertinent information. Takes appropriate action as required.
  • Triages emergency and non-emergency phone calls providing appropriate direction. Documents in EMR according to PCC established protocols.
  • Directs patient flow. Assists other staff in need of assistance, to maintain patient flow.
  • Keeps providers informed of changes in assignments or patient flow that will affect working conditions.
  • Performs laboratory and diagnostic tests as ordered by the provider.
  • Cooperates with other health care personnel to coordinate patient care and establish mutual goals.
  • Notifies patient by telephone, certified mail, or via PCC patient portal of any abnormal test results as directed by provider.
  • Assists clinical staff with the orientation of new RN staff. Assesses educational needs of clinical staff, provides additional training as needed.
  • Initiates and pursues own plan for continued professional growth.
  • Maintains patient confidentiality.
  • Complies with PCC policies, procedures, and safety practices.
  • Maintains a neat and orderly clinic work area.
  • Complies with PCC customer service guidelines and protocols.
  • Coordinates and evaluates daily operations to ensure the delivery of quality patient care and the effective and efficient management of the clinic under the direction of the Clinic Administrator.
  • Plans for long and short-term provisions of supplies and equipment to meet patient needs.
  • Develops work schedules, monitors daily staffing needs and assignments.
  • Responds to physician and patient complaints in collaboration with site leadership
  • Monitors and evaluates patient care by observing and guiding staff performance according to clinic standards of care and practice, policies and procedures.
  • Maintains nurse driven protocols and standing medical orders in collaboration with leadership
  • Supports RN development activities through active participation with nursing staff in the identification of learning needs during new employee orientation, training and continuing education.
  • Drive continual quality improvement through participation in organizational quality improvement efforts that may include but are not limited to patient satisfaction, clinical and operational quality improvement initiatives.
  • Functions as a team lead as assigned. Team Lead duties include:
    • Assists and coordinates with interdisciplinary team leads to maintain patient flow.
    • Monitors all RN team desk tops. Monitors work load of the Team Nurse, assists Team Nurse with duties as needed.
    • Functions as a resource for all clinical and non-clinical support staff.
    • Assigns lunches and lunchtime coverage to RN staff.
    • Delegates work assignments as needed (i.e. nurse visits).
    • Reviews tracking and reporting of RN driven visits
    • Performs other duties as assigned.

Experience/Training:

  • Graduate of an accredited school of nursing.
  • Constantly communicates with patients, families, and other healthcare providers. Must be able to exchange accurate information in these situations.
  • 2 3 years clinical experience preferred.
  • 2 - 3 years EMR experience preferred.

Certifications/Licenses:

  • Current and valid State of Illinois nurses license.
  • Current and valid CPR card.

Physical Demands:

  • Must be able to remain in a stationary position 50% of the time.
  • Must be able to move around the clinic site 50% of the time.
  • Constantly operates a computer, computer printer, copy machine, and telephone.
  • Occasionally positions self to maintain exertion of physical strength to move objects of 10 pounds from one level to another.
  • Ability to move as needed to perform necessary nursing skills such as wound care, injections, and other procedures.
  • Must be able to transport from one site to another.
  • Must be able to cover other shifts as necessary.

Language Skills:

  • Ability to read and write proficiently using the English language.
