Responsible for community-based direct and indirect care given to assigned participants in compliance with state regulations, professional standards of practice, physician orders, and individual health/treatment plans. Provides follow up care and monitoring subsequent to critical incidents, hospital discharges, and ER visits. Administers and monitors prescribed medications and treatments in accordance with approved nursing techniques and physician’s orders. Provides case management to persons with severe mental illness or co-occurring disorders, both on-site and in the community. Recommends and refers participants to appropriate medical services. Documents all services using program standards. Acts as a collaborative member of an interdisciplinary team in agency and participates in risk mitigation planning. Provides back-up coverage to other agency nurses.
Nursing degree from accredited university/college.
Experience working with persons with psychiatric disorders and dual-diagnosis population required.
Valid Illinois Registered Nurse (RN) license required.
Valid Illinois driver’s license, insurance, and daily access to vehicle.
