How Will You Be An Impact?

Directly supervises and develops Nurses and Medical Assistants (MAs) and manages daily clinical activities and productivity while ensuring the provision of high-quality patient care provided by Nurses and Medical Assistants. Serves as PCC site management team member; takes initiative to improve patient outcomes according to evidence-based practices. Models and provides high-quality, patient-centered, trauma-informed nursing care in a team-based approach to achieve mutually established patient goals consistent with the mission and vision of PCC Community Wellness Center.

Anticipated Time Allocation: 20% of time is dedicated to direct/clinical patient care and 80% of time is dedicated to administrative functions as outlined below.

Essential Duties and Responsibilities:

Professional/Leadership
Through therapeutic relationships with patients, families, and colleagues, ensures high-quality patient care provided by clinical support staff and facilitates achievement of the clinical and organizational goals.
Manages the clinics day-to-day clinical operations, including practice standards, staffing, and quality improvement.
Manages, plans, and monitors daily assignments, staffing, and scheduling. Delegates work assignments to Nurses and MAs as needed to balance workload and promote efficient patient flow. Monitors room assignments; makes adjustments as necessary.
Leads implementation of systems that improve patient intake, flow, and patient satisfaction and provides feedback to Director of Team Nursing and other members of the leadership team.
Ensures providers are informed of changes in assignments or patient flow that may affect working conditions. Directs and assists clinical and administrative support staff in need of assistance to maintain patient flow.
Oversees the evidence-based care triage of patients and appropriate documentation of intake activities, screenings, assessments, and plans.
Implements new initiatives/procedures and trains clinical and clerical staff on policies, flowcharts, and/or workflows.
Actively participates in PCC Community Wellness Centers communication processes through timely review and response to electronic and written messages.
Acts as a professional leadership role model; motivates clinical support staff. Responsible for orientation, training, development, and management of clinical support staff.
Increases employee motivation, retention, and engagement by providing ongoing coaching to staff. Designs and implements action plans to improve staff development.
Communicates effectively and maintains confidentiality with patients/families, providers, clinical support staff, and interdepartmental staff. Demonstrates the highest level of customer service to internal and external customers.
Advocates for patients, families, and their significant others, coworkers, community members, as well as the nursing medical assistant roles.
Communicates in an effective and professional manner to reach conflict resolution.
Monitors internal and external customer service. Ensures quality customer service to all, consistent with PCCs customer service guidelines and protocols (such as AIDET).
Promotes collaboration between coworkers, all members of the health care team, as well as patients, their families, and significant others.
Treats patients/families, staff, students, and visitors with respect and dignity.

Organizational
Adheres to and enforces PCC Community Wellness Centers Policies and Procedures.
Adheres to and enforces clinical support staff time, overtime, and attendance policies.
Complies with and ensures completion of clinical support staffs annual PCC Compliance month requirements by established date.
Initiates and participates in investigations, reporting, and resolution of patient complaints and concerns utilizing established communication channels/chain of command.
Identifies and resolves recurring clinical practice and/or related clinical problems.
Orders medical supplies under supervision of the Director of Team Nursing.
Confirms completion of quality control checks on specific medical equipment and clinic-based diagnostic tests following established protocols.
Manages and controls clinics expenditures within establish budgets.
Serves as back-up support for sterilization team.
Functions as site-based EMR Superuser.

Education/Research
Facilitates the planning, presenting, and evaluating of learning/educational experiences for staff and students.
Serves as resource to nursing and medical assistant staff and students.
Directs clinic-based quality initiatives according to Institute for Healthcare Innovation (IHI) standards and provides feedback and education to staff during PDSA cycles.
Encourages and supports nursing and multidisciplinary research efforts conducted at PCC Community Wellness Center.
Responsible for continued clinical support staffs professional growth/development by identification of learning needs. Initiates strategies to meet such needs.

Strategic Planning and Goal Setting
Studies most recent trends and innovative approaches in the ambulatory health care industry (such as AAACN) to apply new concepts towards organizational strategic goals utilizing the IHI model for improvement.
Manages positive relations with providers to facilitate patient care. Oversees provider schedules to ensure appropriate MA coverage.
Participates in site-based leadership team. Attends monthly meetings. Provides primary input to the Steering Committee, Operations Team, Safety Officer, and executive leadership team on decisions affecting practice site(s), as well as feedback on departmental programmatic, planning, and policy issues.
Reviews literature for upcoming HRSA, PCMH, Joint Commission, and Illinois regulatory audits; develops strategic plans to implement prospective changes as needed.
Directs and implements disaster management drills for designated PCC site when needed.
Participates in site-based renovation planning as indicated.
Meets established work/compliance/project deadlines as required.

Regulatory Compliance
Verifies nurses and medical assistants complete all initial/annual competencies and maintain current licensure and certifications prior to deadlines/expiration (i.e. CPR).
Ensures policies, procedures, and workflows are reviewed by nurses and medical assistants at least annually and when updated, as required by PCC Community Wellness Center.
Ensures safe management of stock vaccine and medication supply.
Leads quality and mandated audits (such as VFC), clinical staff Quarterly Metrics, and annual reviews.
Monitors quality of patient care by preparing and reviewing EMR-based reports and occurrence reports and initiates appropriate interventions as needed to ensure patient safety and meet or exceed customer expectations and established standards/benchmarks.

Staff Management
Oversees the proper patient assessment, procedure completion, and appropriate documentation of routine patient care performed by nurses and medical assistants.
Ensures that nurses and medical assistants perform and document basic patient history and administer prescribed medications and treatments as directed by the provider, following PCCs policies and procedures.
Monitors clinical staff interactions with patients to ensure equity in care provision following trauma-informed care model.
Interviews, hires, orients, leads, mentors, and evaluates performance of all clinical support staff under the nursing managers direction to meet departmental and organizational needs.
Directs and assists other site-based staff to maintain patient flow.

Clinical
Completes required orientation program and role-specific competencies.
Identifies patient needs and implements interventions within nursing scope of practice and updates the patient's chart based on ongoing assessment of the patient.
Verifies and implements orders from providers by performing interventions as prescribed and documenting accurately in the electronic medical record.
Engages and obtains information from patient and/or family to develop individualized patient educational plan.
Engages support personnel by delegating appropriate tasks per the Illinois State Nurse Practice Act and established PCC policies, procedures, and workflows.
Demonstrates a commitment to PCC Community Wellness Center through teamwork and effective communication in an effort to achieve quality outcomes, patient safety, customer satisfaction, and a clean, safe, and therapeutic environment.
Functions as expert Nurse and Medical Assistant when necessary.

Basic Qualifications:

Experience/Training
Graduate of an accredited nursing program, BSN preferred.
Previous clinical and EMR experience, 3-5 years required.
Supervisory experience, at least one year preferred.
Proficient in MS Office Business Applications, including: Outlook, Word, PowerPoint, and Excel.

Certifications/Licenses
Current and valid State of Illinois Nursing License.
Current and valid CPR-BLS certification.

Physical Demands
Must be able to remain in a stationary position 50% of the time.
Must be able to move around the clinic site 50% of the time.
Constantly operates a computer, computer printer, copy machine, and telephone.
Occasionally positions self to maintain exertion of physical strength to move objects of 10 pounds from one level to another.
Must be able to transport from one site to another.
Must be able to cover other shifts as necessary.
Required shift rotation.

Language Skills
Ability to read and write proficiently using the English language.
Bilingual English/Spanish preferred/at some locations required.

Expectations
Complies with PCCs policies, procedures, and safety practices.
Maintains a neat and orderly clinical work area.
Performs other related duties and participates in special projects and assignments as assigned.
