Pathways Healthcare is Hiring a Clinical Liaison Near Stoneham, MA

Pathways Healthcare is a leading, physician and nurse practitioner-led medical rehabilitation home health organization dedicated to providing comprehensive care. Our innovative Medical Rehabilitation Program offers patients an exceptional alternative to institutional short-term rehabilitation.
We have an exciting opportunity for a clinical liaison to join our growing team in Stoneham. As a clinical liaison you will evaluate patients referred to Pathways, identify insurance coverage plans and coordinate referrals for home care services.
Position Details:
  • Main Office location: Stoneham, MA
  • Job Type: Full Time
  • Schedule: Monday - Friday
Essential Job Functions/Responsibilities:
  • Evaluates hospitalized patients referred for home care.
  • Identifies and verifies insurance coverage of home health care services.
  • Obtains and documents prior authorization for home care services from insurance providers.
  • Meets performance standards set forth by Director of Marketing in coordination with COO. Performance standards subject to change at any time.
  • Collaborates with the hospital's discharge planning personnel, utilization review department and insurance Case Managers to facilitate safe, early discharge.
  • Consults with physicians, nurses, social workers, discharge planners and other disciplines to establish a coordinated home plan of care.
  • Interviews the patient, family, and caregiver and discusses the home situation, current needs, and any psychosocial factors that are relevant to the plan of care.
  • Completes referral information that includes intake data, essential background information, hospital course, and the plan of care.
  • Maintains liaison relationship with hospital and insurance personnel, providing information and education on Pathways services, coverage issues and related areas.
  • Participates in patient care conferences and in-services.
  • Collects and maintains statistical data on all referrals and submits them regularly as required.
  • Attends meetings and participates on committees as required.
  • Performs other duties as assigned.
Position Qualifications:
  • Experience meeting goals, deadlines and performance benchmarks preferred.
  • Clinical or Healthcare Sales Background Preferred South Shore Area Hospitals
  • Demonstrates an ability to work with other health care clinicians, development of home plan of care, knowledge of third-party reimbursement, and ability to negotiate payment rates.
  • Self-directed and able to work with minimal supervision.
  • Strong ability to cultivate new relationships.
  • Demonstrate excellent observation verbal and written communication skills.
