Boston Medical Center is Hiring a Financial Community Wellness Advocate Near Boston, MA

POSITION SUMMARY:

The Community Wellness Advocate (CWA) is a member of the StreetCred and Pediatrics team who helps promote health and wellness for families, through integrating economic mobility services particularly focused on maternal-child health equity. CWAs are responsible for assisting with collection of baseline needs assessment data, including consenting families for participation and collecting survey data; providing advocacy and case management services; developing and tracking an economic mobility plan based on identified patient needs; facilitating access to and coordination among economic services and other internal and external resources; monitoring the families' progress; and problem-solving with patients to both accelerate and enhance access to concrete supports.

CWAs provide clinic-based one-on-one, family, and/or interdisciplinary group support and collaborate with all members of the care team to conduct needs assessments to identify and respond to barriers to the families' economic well-being in addition to providing economic services as a part of routine well visits. As a member of an innovation model with testing new approaches to build economic well-being, the specific activities of the CWA may evolve over times as the model evolves .

ESSENTIAL RESPONSIBILITIES / DUTIES :

CORE DUTIES

  • Functions a key member of an inter-disciplinary team (nurse, social workers, physicians, clinical support staff, etc.)
  • Collects baseline economic needs assessment data using IRB protocols that include explaining study objectives, obtaining consent, verbally conducting survey with parents and entering data into electronic survey, issuing compensation for time (ClinCards)
  • Maintains high quality data tracking systems in Epic, Excel (in Box), and REDCap, among others
  • Provides parent coaching and support focused on economic well-being, including outreach to and engagement with families to define individual goals and promote and access to resources
  • Meets with families during clinic visits to assess economic resource needs and to connect to specific services
  • Between clinic visits, schedules and completes virtual and/or in person visits to enroll families in specific economic resources (e.g., 529 college savings accounts, Paid Family and Medical Leave, etc)
  • Becomes certified as an IRS tax preparer and prepares taxes during tax season to promote families receiving the Earned Income Tax Credit
  • Assists families in addressing and overcoming barriers with a range of concrete supports including coaching, checking in via text messaging, filling out paperwork, and navigating systems
  • Clearly documents all activities in the patient's record and care management system. Records and monitors the participants' progress toward goals within specific timeframes. Documents assessments and key patient updates in Epic system; documents relevant day-to-day activities and patient data.
  • Communicates plans and updates with members of the care team
  • Develops and maintains strong relationships with economic resources in the community to ensure patient access
  • Engages in ongoing training and learning
  • Provides Trauma Informed Care and case management training and documentation

JOB REQUIREMENTS

EDUCATION:

  • Bachelor's degree required; Master's preferred.

CERTIFICATES, LICENSES, REGISTRATIONS REQUIRED :

  • None

EXPERIENCE:

  • 2-5 years of prior healthcare, public health, mental health, or community-based programmatic experience preferred, with knowledge of MA community services landscape
  • Prior experience with quality improvement projects preferred

KNOWLEDGE/SKILLS:

  • Basic knowledge of healthcare system and social service landscape
  • Outstanding interpersonal and communication skills
  • Understanding of how language, culture and socioeconomic circumstances affect health
  • Bilingual candidates preferred, preferably in English and either Spanish or Haitian Creole
