With a staff complement of 200 strong, we serve a radius of 100 miles around Missoula with a broad array of professional healthcare including nursing, occupational therapy, physical therapy, speech therapy, social work and bereavement support. Our programs encompass Home Health, Hospice (the largest Hospice in the community), Home Infusion, Home Medical Equipment (including home modifications for those who want to age-in-place) and HCBS (the Medicaid “waiver” program that supports those who wish to live with independence).

Our RNs put the patient at the center of care... They work with a inter-disciplinary team to provide skilled nursing care, and case management for patients in their homes. They coordinate services to ensure continuity of care for patients and their families. They manage patient care through a staff of professional healthcare providers including Home Health Aides, Social Workers, Chaplains, and other team members. Through patient and family instruction and evaluation, they provide the highest quality care and attention. They maintain accurate, current and complete patient records to assist with patient outcomes. On-call scheduling is assisted through a first-contact service that we contract with to minimize the disruption in our nurses lives; the service reduces the number of calls by approximately 50%.
This is a full-time position with a business hour visit schedule and a rotating schedule for on-call and holiday coverage. We use a patient triage service for on-call support which handles most patient need during the on-call shift.

Your Qualifications include... A current RN license in the State of Montana (BSN preferred), a valid driver’s license, appropriate auto insurance, and reliable transportation. Two years of nursing experience required. 2 years of recent acute care or home care experience and experience with death and bereavement strongly preferred.
