Who We Are: Partners in Home Care is a nationally accredited non-profit organization that has been providing quality home health care services to patients and families throughout Western Montana since 1987. We are passionately committed to assisting patients and families in achieving optimal health, independence, and comfort through high quality, cost-effective home and community health-related services. With a staff complement of 200 strong, we serve a radius of 100 miles around Missoula with a broad array of professional healthcare including nursing, occupational therapy, physical therapy, social work and bereavement support. Our programs encompass Home Health, Hospice (the largest Hospice in the community), Home Infusion, Home Medical Equipment (including home modifications for those who want to age-in-place) and HCBS (the Medicaid “waiver” program that supports those who wish to live with independence).

Job Summary:

The RN – On-Call provides skilled nursing on-call coverage, including availability for skilled nursing visits and admissions, for the Hospice and Home Health programs. On-call coverage is provided from 5:00 p.m. to 8:00 a.m. according to an established schedule. The On-Call RN will answer incoming calls for the home health and hospice programs from Missoula, Hamilton and Lake branches, but will not perform visits for the Lake branch.

Essential Duties and Responsibilities:

1. Provides skilled nursing and coordinates hospice and home health care to patients and family members in a residential setting through a team approach and based upon systematic evaluation as authorized by a physician.
2. Conducts skilled nursing visits and admissions during on-call hours.
3. While on-call, conducts triage of all HH and Hospice patient needs via telephone, provides advice and/or makes home visits for Missoula branch patients as deemed Provides hand-off to Lake Co on-call nurse to perform visits when necessary.
4. Assures continuity of care through communication with case managers, other disciplines, and appropriate agencies. Follows-up to ensure thorough communication with managers and other on-call staff.
5. Promotes optimal health and quality of life for patients through teaching, and, appropriate nursing interventions.
6. Observes and evaluates the physical and psychosocial status of patients, including changes in condition and/or reactions to treatments or medications.
7. Assist Clinical Managers with back-office Home Care Home Base tasks such as, but not limited to, running and reviewing reports, assigning visits and care team members.
8. Attends in-services and other meetings as required.
9. Performs other duties as assigned by supervisor.

Minimum Qualifications:

Qualifications include a current RN license in the State of Montana (BSN preferred), a valid driver’s license, appropriate auto insurance, and reliable transportation. 2 years of recent acute care, home care, or hospice experience including such clinical skills as symptom management, catheterization, and wound care preferred. High speed Internet access while working from home is required.

Physical Demands & Working Conditions:

Work is performed in both the office environment and in the residences of patients, requiring driving in varied weather conditions and often at night. Occasional medium lifting is necessary. The position also includes regular use of a keyboard, and use of hands to finger, handle, and feel. Position involves frequent exposure to blood and bodily fluids.
