Targeted Case Manager, Employee Position

Parsons Counseling & Play Therapy Center is seeking targeted case management coordinators (TCMs) to provide resources for children, teens, and adults. Ideal candidates are service oriented, have a strong sense of personal ethics and integrity, and seek a collaborative and supportive work environment. All positions are for W2 employment.

Position Description

TCMs assist clients in gaining access to needed medical, social, educational, or other needed services and supports, including: (a) Assessment of the client’s medical, social, and functional status and identification of the client’s strengths and needs; (b) Arranging for service delivery from the client’s, the client’s legal guardian, or the client's custodial parent’s chosen provider to insure access to required services; (c) Facilitating access to needed services by explaining the need and importance of services in relation to the client’s condition; (d) Facilitating access, quality, and delivery of necessary services; and (e) Preparation and maintenance of case record documentation to include care plans, forms, reports, and narratives as appropriate.

Services may be home or school-based. Some travel is required. We serve Madison county and the surrounding area, with locations in Berea and Richmond, KY.

Position Responsibilities

  • Provide quality support services that are in accordance with agency, state and professional standards
  • Complete clinical documentation in a timely manner
  • Participate in staff meetings and in-service training
  • Coordinate with other staff and the services director
  • Complete additional training as required by the state

Minimum Qualifications

  • Possess a bachelor of arts or science degree in a behavioral science field
  • Psychology;
  • Sociology;
  • Social work;
  • Human services;
  • Counseling;
  • Nursing;
  • Behavioral analysis;
  • Public health;
  • Special education;
  • Recreational therapy;
  • Education;
  • Rehabilitation counseling
  • Have at least one (1) year of full-time employment experience after completing the educational requirements:
  • 1. Working directly with adults in a human service setting; or
  • 2. Working directly with individuals under the age of twenty-one (21) in a human service setting;
  • Complete a successful criminal background check
