Manhattanview Rehab Center,a 12-bed skilled nursing and rehabilitation facility located in Union City, New Jersey,is in need of Full Time Social Worker to assist the Director of Social Services. Candidate must have a current NJ Social Worker certification or license in good standing.

Duties include, but are not limited to:

·Develop and implement social service policies and procedures to meet residents’ needs in compliance with federal, state and local regulations.

·Develop, coordinate and participate in family and resident activities designed to promote social interaction, reality orientation and intellectual stimulation.

·Counsel residents and families in dealing with feelings about death or dying and other emotional, mental, environmental or physical limitations.

·Assess and complete the sections of the (MDS) Minimum Data Set assigned completely, timely and accurately.

·Document the social service component of the Comprehensive Care Plan for each resident in a timely manner.

·Document psycho-social needs, interaction and follow-up action(s) with resident and/or family according to facility policies and procedures.

·Assess, plan and document residents’ discharge needs in accordance with facility discharge planning policy and regulatory requirements.

·Act as a liaison with social, health and community agencies.

·Refer residents to social, health and community agencies and complete accurate documentation in residents’ records concerning the results of such referrals.

·Coordinate family and community resources as needed to solve financial needs and to promote emotional security.

·Explain residents’ rights to residents and residents’ representatives.

·Participate in admission of residents as assigned

·Plan, schedule and implement room visits for residents unable to leave their rooms.

·Organize and participate in the theft loss program.

·Be responsible for the safety of residents under his/her supervision.

·Demonstrates knowledge of and respect for the rights, dignity and individuality of each resident in all interactions.

·Appreciates the importance of maintaining confidentiality of resident and facility information.

·Demonstrate honesty and integrity at all times in the care and use of resident and facility property.

·Comply with all facility policies and procedures. 

·Additional duties as assigned by the supervisor

QUALIFICATIONS:

• Must have long term care experience
• Must have prior experience as a Social Worker
• Must have LCSW or CSW
• Strong verbal & written communication skills
